It typically presents as a rapidly progressive dementia (RPD). For a definitive diagnosis, brain biopsy or autopsy is required (definite CJD). Premortal diagnosed patients are called probable CJD according to the diagnostic criteria of Zerr et al. So this entity can be difficult to identify antemortem.
