The Community Health Agents Programme (PACS) is a programme that offers individuals diagnosed with Systemic Arterial Hypertension (SAH) opportunities for registration, follow-up and treatment of the disease. This research is a descriptive, cross-sectional study of living and health conditions, risk factors, complications, expectations and difficulties faced by individuals diagnosed with the aforementioned disease, registered and monitored by the PACS in a municipality in the state of Minas Gerais. The results made it possible to create a scenario of the reality studied and provided input for discussion and the development of individual and collective actions within the health system to promote the health and well-being of these individuals. This has allowed for a better perspective on the conditions of treatment, monitoring and control of the disease, as well as anticipating the presence of missed opportunities in health care and some reflections on the possible harm of this proposal for providing care to the population.
